With your permission, we may adapt your story into a short narrated reel. We always change names, places, and any details that could identify you. Your words become a message of hope, and you stay anonymous.

I give Forever Blessed permission to use my story. I understand that Forever Blessed may edit, shorten, and adapt my story into scripts, short videos ("reels"), newsletters, social posts, and other content, now or in the future, on any platform. I understand my story will be retold anonymously: names, places, ages, and other identifying details will be changed. Forever Blessed's reels use narrated voices (these may be digitally generated voices) and illustrated scenes, so the reel will not be my voice or my face. I confirm the story is my own experience, that I'm 18 or older, and that I haven't included anyone else's private information without their permission. I understand this is free and voluntary, that I won't be paid, and that Forever Blessed doesn't have to use my story. I can ask Forever Blessed to stop using my story by emailing weareforeverblessed77@gmail.com. They will stop making new content from it and will remove posts they control where reasonably possible, though copies other people have already shared may remain. Submissions are stored with our email or form provider and in a private working file that only the Forever Blessed team can see. We delete raw submissions we don't use after 12 months, and you can ask us to see, correct, or delete your information anytime at weareforeverblessed77@gmail.com.
